Here is the information you need. Just send a money order for 5 dollars, state your full name, date of birth, parents names and place of birth. Also, send a copy of a state issued identification card. If you do not have a driver's license obtain a state ID at your state motor vehicle dept or send a copy of a school identification card.

SAN JUAN (Central Office)
Departamento de Salud, Registro Demográfico
P.O. Box 11854
Fernández Juncos Station
Ave. Ponce de León #1906
San Juan, PR 00910
787-767-9120 ext.2315

Central office has had records since July 22, 1931. Copies of earlier records may be obtained by writing to local Registrar (Registrador Demográfico) in municipality where event occurred or by writing to central office for information.

To obtain certified abstracts of vital records, send your request, a copy of your photo ID, and either a $5 Internal Revenue stamp or a $5 money order payable to the Secretary of the Treasury for each record requested to the registrar. Please contact your registrant office for further information and/or to confirm fees (Puerto Rico area code is 787).
